DUTIES:
GENERAL DESCRIPTION

The Mental Health Intensive Case Management (MHICM) Nurse provides direct and indirect psychiatric and medical nursing care to Veterans and their families working independently in the community. The incumbent registered nurse will be assigned to the Behavioral Health Services (BHS) Department and will be directly accountable to the Wilmington VA Medical Center (WVAMC) Chief, BHS and/or BHS Nurse Manager.

The MHICM Nurse is responsible for facilitating the fulfillment of the Wilmington VA Medical Center Mission, Vision, and Values. The incumbent is responsible for the provision of quality, customer-focused and cost effective patient care within the MHICM program of Behavioral Health services at the Wilmington VA Medical Center. The MHICM Nurse provides direct patient care in the community that includes assessing, planning, health promotion, and preventive. The incumbent collaborates with the MHICM Clinical Specialist and Social Worker in the screening and evaluation of the Veterans for the MHICM Program. The incumbent also works collaboratively with Veterans, families, and interdisciplinary treatment team members in the provision of care. The incumbent acts as a liaison to the inpatient, outpatient and community agencies involved. The incumbent will be responsible for completion of required clinical records as well as MHICM forms for evaluation and monitoring of service delivery outcome. The incumbent will further be responsible for providing data and information for the completion of semi-annual reports as requested by the Northeast Program Evaluation Center (NEPEC).

QUALIFICATIONS

The MHICM Nurse is a citizen of the United States and is currently licensed as a Registered Nurse with an active, current, full, and unrestricted license. The incumbent is a graduate of a state approved nursing school or accredited by either the National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). The incumbent has an Associate Degree and approximately two to three years of successful nursing practice focusing on psychiatric nursing or has a Bachelor of Science in Nursing (BSN) with one or two years of successful nursing experience with a focus on psychiatric nursing.

FUNCTIONS

The MHICM Nurse collaborates with the MHICM Social Worker in the screening and evaluation of Veterans for the MHICM Program.

Act as liaison to the inpatient, outpatient, and community agencies involved in the provision of care to the identified Veteran population. Works collaboratively with Veterans, families, and interdisciplinary treatment team members in the assessment planning, implementation, and evaluation of treatment needs.

Participates in coordination and facilitates utilization of community resources.

Performs comprehensive assessment of biopsychosocial needs of Veterans and forms a diagnosis using the DSM-IV-TR.

Develops treatment plans with specific goals and interventions unique to each Veteran. Provides individual, group, and family psychotherapy to Veterans and families when needed.

Participates in home health care delivery. This is accomplished by home visits to identified Veterans and families to include comprehensive assessment and planning of service needs.

Participates in the response to psychiatric crises, assessing crisis situations, formulating crisis plans, and providing follow-up interventions. When indicated, facilitates admission to psychiatric inpatient services.

Under the supervision of the treating physician facilitates orders for laboratory tests, procedures for non-invasive tests, consultations, and medications.

Will provide late hours on some weekdays and on Saturdays, as needed.

Other duties as assigned

ADMINISTRATIVE

Collaborates with other health care professionals in the development and evaluation of MHICM. Participates in relevant nursing and medical center committees, task forces, and/or groups.

EDUCATION

The MHICM Nurse furthers his/her knowledge and expertise through the ongoing assessment of learning needs in relation to professional development. The incumbent participates in the appropriate learning activities to fulfill identified needs.

Participates in patient and family education. Assesses patient/family educational needs and provides education to patients and families specific to those needs.

Provides formal and informal inservice education to interdisciplinary health professionals. Facilitates the education process for students by serving as a preceptor to undergraduate/graduate nursing students.

CONSULTATION

Serves as a consultant to the psychiatric service and other hospital providers in the area of community based psychiatric case management.

Acts as a consultant and resource person to outside agencies, e.g., residential living facilities in the area of psychiatric nursing care delivery.

QUALIFICATIONS REQUIRED:
The Staff Nurse is a citizen of the United States and is currently licensed as a Registered Nurse with an active, current, full and unrestricted license. The incumbent is a graduate of a state approved nursing school or accredited by either the National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). He/she has an Associate Degree or Diploma and approximately two to three years of successful nursing practice or has a Bachelor of Science in Nursing (BSN) with two to three years of successful nursing practice; or a master's degree in nursing with one to two years of successful nursing practice; or a master's degree in a field related to nursing and a BSN from an accredited or state approved school of nursing with one to two years of successful nursing practice; or a doctoral degree in nursing or related field. He/she has the physical and mental capability to satisfactorily perform assigned duties. Maintaining a current BLS certification is required.

In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Must be proficient in spoken and written English .

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research.

Education/Experience Requirements:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience. Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
