This is a full time opening.

To keep pace with the changing health care environment and build a healthier future for our children, Nemours has embarked on a multi-phase expansion to create a children’s hospital l! ike no other in the region. Upon completion in June 2014, the ! hospital will include private, family-centered, technologically advanced patient rooms, a new Pediatric Intensive Care Unit, and a new expanded Emergency Department.
The Advanced Practice Nurse is responsible for an area of specialized nursing practice, and provides direct and indirect care to patients and their families in an Outpatient primary care setting.
ESSENTIAL RESPONSIBILITIES
Practices collaboratively with physicians and other health care providers to provide high quality health care to children.
Performs appropriate assessment of patient/family.
Performs ongoing assessment and evaluation o f patient progress and revision of interdisciplinary plan of care.
Identifies patient/family need for further resources and initiates appropriate referrals.
Documents patient care and progress on patient’s chart in Electronic Medical Record.
Provides patient/family teaching based on identification of lear! ning needs.
Evaluates compliance with the prescribed treatment regimen through telephone calls, family/patient assessments and outpatient visits as needed.
Utilizes advanced knowledge and specialized skill in nursing practice in accordance with the Nurse Practice Act, hospital policy and protocol
Education, Training and Experience
Master of Science in Nursing.
Five years of related clinical experience
Pediatric Primary Care experience preferred
Licenses, Registrations, or Certifications
Current Registered Nurse license in the State of Delaware,
Current Advanced Practice Nurse license in the State of Delaware, with prescriptive authority.
