General Description:
Christiana Care Health System is currently seeking a full-time Nurse Practitioner for our Medical Home Without Walls department.
In collaboration with the program Medical Director, the Nurse Practitioner (NP) functions as a self-directed practitioner who provides leadership and facilitates coordination of services for patients along the continuum of care. Utilizing advanced assessment, planning, implementation and evaluation skills the NP develops patient-centered goals, provides comprehensive patient care and education, and works to engage patients in proactive self-management of healthcare needs.
Elicits detailed health histories, performs physical exams and screens patients to identify abnormal problems and records findings. Initiates or modifies treatment and medication within established guidelines. Initiates requests for essential laboratory procedures and radiologic studies as needed and collects, performs and i! nterprets lab results as required. Plans and oversees on-going! treatment and clinical management of patients in stable phases or chronic illness. Initiates referrals to appropriate resources and services, promoting a collaborative approach to patient care. Prescribes medications as credentialed by the Joint Practice Committee of the Delaware Board of Medical Practice. Writes orders and documents in the medical record as indicated. Consults with physicians regarding patients’ conditions, diagnosis, treatment plan, medication orders and need for specialty care. Keeps physician advised of patient’s status as necessary. Provides emergency medical treatment when necessary. Informs, counsels and instructs individuals and families on their specific health problems and test results. Prepares and maintains complete and accurate medical records and other documents. Attends, participates in discharge planning and multi-disciplinary case conferences and/or family meetings to promote high quality of care. Analyzes documentation and feedback to! facilitate problem resolution.
Requirements: Graduate of an accredited Nurse Practitioner Program. Must have DE RN and APN license or be license eligible. 2-3 years previous Home Care or Community Health experience required.
