POSITION DESCRIPTION: To encourage & assist residents to attain/maintain their highest
leve! l of function, autonomy, & independence.
Assist in admission, discharge or transfer of residents
Take & record heights & weights
Assist residents in maintaining activities of daily living (personal hygiene, bathing, oral care, foot & nail care, Dressing)
Pass meal trays & assist residents as needed with meals, feed if required
Pass nourishments & ice water
Escort residents to meals, activities & appointments if assigned
Record intake and output
Assist residents with toileting needs
Assist residents with transfers (beds, wheelchairs, gerichairs)
Answer call bells in a timely manner
Keep residents room(s) and service areas clean, orderly and safe
Distribute & change linens as needed
Provide range of motion & ambulation. Turn & reposition residents as needed.
Help maintain privacy & personal dignity of residents
Provide postmortem care to deceased residents
Communicate changes in resident condition to license! d staff
Complete necessary paperwork in a timely manner (A! DL flow books, meal percentages, etc.)
Participate in inservices, training, and meetings
Other duties as assigned by licensed staff
Requirements
Job Requirements
Education:
Delaware C.N.A. certification
Experience:
Prefer a minimum of 1 year C.N.A. experience
