Quality Insights of Delaware (QID) has an immediate need for an experienced RN to step into the role of RN Project Coordinator within its CMS Medicare Quality Improvement Organization (QIO) designation. This position is responsible for demonstrating proficient and advanced clinical skills in planning, implementing, and evaluating cooperative projects that have long-range implications for select patient populations in various clinical settings. Coordinates internal and external resources for execution of cooperative projects including interdisciplinary management of projects within and outside the organization. Effectively assists health care providers (specifically nursing homes) and organizations in problem solving, including serving as an external resource for quality-improvement data and performance-improvement strategies. DUTIES

Serves as primary liaison for health care providers on cooperative projects under his/her direction.Provides guidance and resources to health care providers on selected topics designated for improvement.Collaborates with directors and scientific support staff on project design, implementation and evaluation.Serves as a resource, within and outside the organization, on standard of care, quality-improvement processes and quality-improvement data.Documents all information related to contract deliverables in appropriate databases for projects under his/her direction.Consults and assists Communications Department with the preparation of information for articles, brochures, newsletters, or other publications.Facilitates educational programs for health care providers.Coordinates seminars to introduce selected health care topics to providers and/or to promote awareness of them.Provides clarification and instruction to providers regarding medical record abstraction processes and developing performance improvement strategies.Ability to identify and recruit new providers to participate in identified strategic health care projects.Manages multiple, parallel projects using formal project-planning techniques.Trains new and existing staff on related task objectives.Assists in troubleshooting potential and/or current problems in achieving contract objectives.Periodically attends seminars on current health-care issues.Develops training aids to assist staff in troubleshooting problems.Maintains a positive customer-service relationship with health care providers through professional interaction and communications.

QUALIFICATIONS

Education: Requires a BSN degree from an accredited college or university, with a MSN preferred.Experience: Five (5) years full-time professional experience in clinical nursing, with at least three (3) years associated with geriatric patient care preferably in nursing home systems, and two (2) years of which must have been in a quality-improvement, program administration capacity.Substitution: One (1) additional year of full-time experience in a program administration capacity, above the minimum set forth, may be substituted for a BSN degree. Post-graduate training in the area of assignment may substitute for the quality improvement experience only. RNs who hold a 3-year diploma from an accredited school of nursing may also be considered if they have a minimum of five (5) years of documented full-time experience in a quality-improvement project administration capacity, three (3) of which must have been associated with the geriatric patient population, specifically in nursing homes.Special Requirement: Must have a current active license to practice as a professional RN in the state of Delaware. Knowledge of MDS 3.0, Nursing Home Quality Measures desirable. Must have knowledge of best practices for nursing home systems and processes related to: resident-centered care, medication safety, reducing antipsychotic medications, dementia care, falls, restraints, and pressure ulcers.
