Description: Delaware Hospice is hiring FT Staff RN NCC right now, this vacancy will be placed in Delaware. Detailed specification about this vacancy opportunity kindly read the description below. The schedule for this position is Monday-Friday 8am-430pm with evenings and weekends, as needed.
POSITION OVERVIEW:
Functions as a member of the interdisciplinary team to provide Hospice services to patients a! nd families. Utilizes the nursing process to provide quality nursing care to Hospice patients in accordance with Delaware Hospice policy. Supports the primary care giver in the performance of physical care for the patient. Active participant in the On Call schedule as needed. Must be able to bend, squat, drive for extended periods of time, lift and turn patients; lift 30 to 60 lbs, following industry standards and guidelines for safety and correct ergonomic practices. Employees involved in patient care may be exposed to blood, body tissue and fluids regularly and are expected to follow all health care industry safety guidelines indicated in our procedures manual. This is an exempt position.

QUALIFICATIONS:
Current license to practice professional nursing in the State of Delaware. (Note: New Castle County RNs must maintain PA license as well). Valid! , Driver’s license that meets guidelines as outlined in the ! Policy. Graduate of an approved school of nursing. Bachelor's degree in nursing preferred. Certification in hospice nursing preferred. Minimum of two years of nursing, preferably in hospice or home care.
1. Utilizes the nursing process to gather data, identify problems, and seek solutions to clinical problems. Assesses, plans nursing care and is responsible for case management involving the patient and family appropriately.
2. Reports and collaborates with Primary Physician and Medical Director as indicated by care needs.
3. Reports and collaborates with Care Manager to assure quality care is provided in accordance with Plan of Care.
4.. Responsible for complete, timely documentation for all visits and supervision of CNAs and contracted Nursing staff .
5. Participates in the interdisciplinary approach to patient/family care. Completes documentation as required by policy and procedure.
6. Ensures completion of Competencies and other reg! ulatory paperwork required.
7. Responsible for participating in the On-Call schedule to ensure that coverage is maintained and we continue to provide high quality care for patients and families 24/7.
8. Participates in activities designed to provide professional growth for self and others.
9. Functions within established policies and procedures to assure compliance to federal and State regulations.
10. Maintains skills required for provision of effective, high quality nursing care which can include phlebotomy, catheter care, stint care, ostomy care, CADD pumps, initiating IVs, managing central lines, tracheostomy care and other skills as identified in the course of providing care.
11. Learn and maintain skills for palliative care, and symptom management as it relates to end of life care.
12. Establish and maintain effective working relationships across the organization including clinical and non-clinical staff.
