Care Link's Care Coordinator RN is an exceptional opportunity for an experienced professional Registered Nurse Case Manager to work in our Wilmington C enter for Virtual Health.
The Care Coordinator RN will be part of a dedicated, telephonic team integrated within the core operations of the Center for Virtual Health.
The Case Manager will be responsible for collaborating with patients/families, physicians, staff, and other health care professionals across the continuum to provide care coordination.
Due to Care Link's geographic reach, candidates must have DE or MD license upon hire, and be willing to obtain PA and NJ licenses within 90 days of hire. Department offers reimbursement for additional licenses, as well as CEU credits.
Requirements: Bachelor's degree in Nursing is required. Candidates who are enrolled in a Bachelor's degree program with 50% degree requirements completed may be considered. ? Active Delaware nursing license is required upon start of employment. Willingness and ability to obtain Pennsylvania and New Jersey nursing licenses within 90 days of hire required. Wi llingness and ability to obtain CCM certification within 18 months of eligibility required. 3 years of nursing experience required. Previous Case Management/ Utilization Management /Homecare experience preferred. Candidates must have excellent teamwork and assessment skills, be proficient at developing the CM plan of care, and the demonstrated ability to manage a large caseload of patients very efficiently.
This is a primarily telephonic Case Manager position located in Wilmington, DE.
