Job Description:

The Prenatal Nurse/Medical Nurse (RN) assesses and assures appropriate disposition of care for patient health problems and needs via telephone triage for routine medical problems, urgent care, prescription refills and requests for information, and also provides care management and in-office clinical care to prenatal, postpartum and newborn patients in collaboration with the clinical team.


CORE RESPONSIBILITIES

1.Assure appropriate disposition of care via telephone triage for routine medical problems, urgent care,prescription refills, and requests for information, following established standards, protocols, and clinicalpractice guidelines

2.Independently complete prenatal intake appointments, STD treatment visits, and nurse education visitsusing established protocols

3.Maintain timely, accurate, complete and detailed reports and records of patient health information inelectronic medical records including the prenatal database, assisting with grant reporting purposes andquality measures as needed

4.Follow organizational guidelines regarding the use of the Electronic Health Record (EHR) in compliance withHIPAA and patient confi dentiality standards

5.Review clinical charts for all active prenatal patients monthly or more often as indicated and ensure timelypatient follow-up

6.Conduct monitoring and follow-up of critical lab values such as bilirubin tracking and PT/INR

7.Perform patient care duties in accordance with established policies and procedures

8.Assist in the development of and/or teach childbirth, parenting or other support classes and help to identifypatients who would benefit from enrollment

9.Assist with chart reviews and audits to ensure compliance with the health care plan

10.Work with individuals, groups, or families to plan or implement programs designed to improve the overallhealth of communities

11.Collaborate with physicians, social services coordinators, medical assistants, receptionists, scheduling, co-workers, and community resources as a patient advocate for coordination of health care services

12.Participate in continui ng education activities to meet state board of nursing licensure requirements

13.Assist in the development of policies and procedures related to clinical practice guidelines
Required Experience:

MINIMUM OBJECTIVE QUALIFICATIONS

1.Graduate of an accredited college or school of nursing with an Associates’ degree in nursing

2.Current registered nurse licensure in the State of Delaware

3.Current basic life support certification

4.Experience using computers (Microsoft Word, Excel, Outlook) and electronic health records software

PREFERRED OBJECTIVE QUALIFICATIONS

1.Bachelors of Science in Nursing

2.Clinical training and experience in ambulatory care and/or prenatal care

3.Fluency in written and spoken English and in spoken Spanish, including medical terminology
