This position exist to assist the Director of Health Services to provide support to community support teams responsible for supporting people wit! h developmental disabilities and to assist to monitor health care services of these individuals. Responsibilities include but not limited to: scheduling medical appointments and coordinating care with interdisciplinary team; assess and implement health care plans; assist with monthly program audits; provide training in the area of medication administration; documentation; participation in meetings and appointments as necessary; ensure compliance with applicable state, federal, agency and licensing regulations.
Full time, exempt position; Schedule varies according to program needs. This position is based in Newark, DE but does involve travel to different locations within New Castle County, DE.
Minimum Required Qualifications:
Must be Registered Nurse with current Delaware State licensure (or eligible via Compact agreement) with minimum of two years experience in human services field/long-term care or medical-surgical setting. Must posses the abil! ity to communicate both verbally and in writing; read, interpr! et, and apply written information such as policies, person centered plans, clinical research findings, and psychological and other therapeutic assessments; able to pass child abuse, state police and FBI clearances; possession of a valid drivers license and be able to drive; ability to transport self and individuals to appointments as scheduled; physically ability to lift to participate in activities of daily living.
